This MyIR portal was developed through a partnership between Scientific Technologies Corporation (STC) and the Maryland Department of Health (MDH) with support from the US Department of Health and Human Services (HHS) and the Centers for Disease Control and Prevention (CDC). Consumers can access their vaccination records through this MyIR portal that pulls the records from ImmuNet, Maryland's Immunization Registry. If you believe the vaccination records you received from MyIR/ImmuNet is incorrect, please contact your healthcare provider to update the vaccination information. Your Immunization Record May Not Look Complete

There may be some vaccines you or your family received that don’t appear on the immunization record in MyIR, such as the human papillomavirus (HPV) vaccine. HPV vaccine is sometimes given by a healthcare provider confidentially. Since MyIR doesn’t know which HPV vaccines were given confidentially or not, all HPV vaccinations are hidden on immunization records in MyIR. Contact your healthcare provider if you think you need a more complete record of your or your family’s immunization history.
